Completed forms should be delivered to one of our offices located at 448 E. 1st St. Salida or 114 Linderman Ave, Buena Vista.

Application
Download a Chaffee Foster Care application or pick one up at 448 E 1st St. in Salida, Colorado. Completed applications must be returned in person.

Handbook
Download a Resource Provider Handbook or pick one up at our office for detailed information on becoming a foster parent in Chaffee County.

Physical
Every potential foster parent needs to complete a physical exam for approved health authorization. Download a Physical Exam Form.

Safe Study
A safe study involves completing Questionnaire 1 – Single Applicant or Questionnaire 1 – Couple Applicant and engaging in an interview with a certification worker where you’ll read and sign the safe study form.

Background
Background checks include signing an Authorization to Release Background Checks, scheduling and submitting fingerprints with a certification worker, and completing a home inspection.

Education
Potential foster parents must attend 27 credit hours of classes, which include the Foster Core Parenting Class, Impacts of Trauma, Reasonable and Prudent Parent Standard, Safe Sleep (for infant caregivers), and Emergency Preparedness.

Photos
Chaffee County Foster Care requires a current photo of all family members living in the household.

Documents
Chaffee County Foster Care requires copies of driver’s licenses, auto insurance, and auto registration.
